Members who are unable to attend meetings of the
membership may still have their voices heard by proxy. In some
cases, specific proxy forms will be mailed out to members along with
meeting notices, as required in the CHPOA bylaws. As an
alternative, an online version of a general proxy form can be
downloaded from this site. The downloaded form can then be
printed, completed, and delivered to the CHPOA Secretary.
Completed proxy forms must be received prior to the meeting for
which they are in effect.
The online proxy form is published as an Adobe PDF
document and requires Adobe Reader software to open. Adobe
Reader is distributed freely and can be downloaded by clicking on
the "Get Adobe Reader" icon below.
